To coordinate wellness care, labs, hormone optimization, peptides, or follow-up services.

Payment

To submit claims for labs or process payment for services.

To evaluate and improve our services, manage internal operations, or conduct quality assessments.

Other Uses Permitted by Law

  • Public health activities

  • Legal or regulatory compliance

  • Workers’ compensation (if applicable)

  • When required by law, court order, or law enforcement

We do not sell your information.

4. Uses Requiring Authorization

  • Most sharing of psychotherapy notes
